Please note, this consultation form is not an actual appointment, once you fill out the form I will respond via email and we can move forward with the booking process. 

In order for me to fully book you for an appointment, I require a deposit which will be taken off the final cost of your tattoo. Please understand many hours of work can go into preparation prior to your appointment, for this reason all deposits are final and nonrefundable.

As a general rule, I do not send out artwork via email before our scheduled appointment. Any changes (within reason) can be made the day of your appointment. Some redesigns may need more time and we might have to reschedule.

I require 48 hours notice to reschedule any appointment, lack of appropriate notice may forfeit your deposit and require a new despot be placed to rebook. Of course, illness or emergency will be considered.
